Planning ahead will enable you to ensure that you have your say in the decisions that affect you. Planning will also make life easier for you and your family and friends by helping you to avoid legal problems in the future.

Advance Care Planning
Advance care planning is a process enabling a patient to make decisions about his or her future health care in consultation with their health care providers, family members and other important people in their lives.  An Advance Care Plan is one way of putting your plan into writing. The written Advance Care Plan helps people to accurately remember what you want and makes it easier to communicate these wishes to doctors and nurses who do not know you.
