These cards are available at no cost and can be obtained by calling the National Dementia Helpline on 1800 100 500.
Card for People with Early Stage Dementia
This card is particularly useful for people who are in the early stages of dementia, and might need help and understanding at certain times, e.g. to show a bus driver who is getting impatient.

Based on a Help Card developed by Alzheimer's Scotland - Action on Dementia.

Personal Identity Card
If a person with dementia is unable to use the Card for People with Early Stage Dementia, the Identification Card could be used. A family member or a carer should fill in the details on this card and ensure that the person with dementia carries it.


Companion Card
Available to anyone who is a carer of someone with dementia. The Companion Card is helpful if a situation arises in public where the behaviour of the person with dementia may not be understood. As it may not always be possible or appropriate for a carer to provide an explanation of the behaviour, the card can be handed over.

The cards are discreet, may be carried in a wallet and include the Helpline contact number to encourage those who receive these cards to contact the National Dementia Helpline 1800 100 500 for further information.
